Adopt A Primary Health Center Initiative
Call for Applications to Adopt a Primary Health Center in rural areas in Nigeria (2024-2026 cycle) -
NANNNA to adopt six (6) PHCs in addition to the NINE (9) adopted since 2019.
In 2024, NANNNA will accept project applications aimed at adopting six primary health centers in Nigeria. The maximum amount of the grant is US$ 6,000 for each PHC for a total of US$ 36,000. The application should be submitted online on or before June 30, 2024.

Introduction
In line with its core mission of improving the health and well-being of Nigerians at home and abroad, NANNNA recognizes the importance of primary health care in ensuring quality comprehensive care, ranging from health promotion and disease prevention, to treatment, rehabilitation, and palliative care at the grassroots level.
On an ongoing basis, NANNNA members volunteer their time and expertise to help their communities by conducting frequent education/health missions to Nigeria, among other initiatives. This initiative is called “Take Me to Your Village.” However, to transform these missions from just missions to a platform for improving health care delivery in Nigeria, the Adopt a Primary Healthcare Center (Adopt-A-PHC) initiative was introduced in the year 2019 in furtherance to the commitment of improving rural health and building on our community health outreach success stories.
At the closing ceremony of its 13th annual scientific conference, NANNNA announced the adoption of six additional Primary Healthcare Centers (PHC) to accelerate primary health care in Nigeria.
Adopt Primary Health Center Initiative
The adoption of primary health care approach is based on our Federal Medical Center, Umuahia, Abia State “Center for Nursing Excellence” success story that uses a consistent approach for better outcomes.
The Initiative focuses on five areas:
(1) Improvement of environment of Care:
(2) Capacity building of health care providers
(3) Data management and record-keeping
(4) Engagement of partners, including community engagement
(5) Telehealth
Criteria for PHC Adoption
- The primary health care (PHC) must be an existing part of the Local Government PHC.
- The PHC must have staff allocated to it by the government.
- Have commitment from the community leaders (the king or community development committee leader), the PHC Medical Director and the Nurse In charge of the PHC, Local Government, and State Government.
- Complete NANNNA Adopt-A-PHC Initiative Application.
- Submit a letter of active participation from chapter president.
- Submit presentation about the community and the PHC (max of 10 slides or short video of 5 minutes).
- Commitment from NANNNA members adopting the PHC (invested interest – two years documentation of work with the community/PHC). One or more of the leads would need to physically visit and work with the local team for at least a week per year.

- 2. Proposal Summary (Executive Summary – max of one page) *
The proposal summary should be a maximum of one page and should include the amount of funding requested and give the most general description of the use that will be made of the funds.
- 3. About Local Government/Primary Health Center History (max of two pages)*
The Local Government/Primary Health Center description and history section should be about one or two pages in length and should include the history of the LG/PHC, its structure, available resources, information about office locations that will be involved in carrying out the activities that will be funded by the requested grant, major contributions of the LG towards the PHC, information about prior grants received from various sources , and an explanation of how the description you provide makes your PHC an appropriate grantee.
- 4. Background /Needs Assessment (max of two pages)*
The Background section, a maximum of two pages in length, should provide an explanation of the problem that has created the need for the PHC to be funded by the requested grant. It should provide evidence that the problem exists as well as how the proposed project will contribute to a solution to the problem or will reduce the harmful impact of the problem. It should highlight media and government publications suggesting that the problem is a high priority. It is important to explain why your PHC should be funded over others.
- 5. Project Description (Program Narrative- max of five pages)*
The project description should give a detailed description of the aspect of the PHC that will be funded by the requested grant. This description should explain the duration of time during which the funds will support the project, the goals of the project, how they will be achieved, how success or failure will be measured, what services you promise to deliver to what population and what results you expect to bring about. A useful structure is to break the project down into component goals. Use each goal as its own heading and under each goal heading, list and describe the activities that will be funded to achieve that goal and how achievement of that goal will be measured or defined. The project description should also include information about the staff who will work on the project, their experience, and qualifications to perform the activities that will be funded. Provide how you plan to sustain the project with funds from other sources.
