Position Description

Job Title: Registered Nurse - Nurse Navigator
Shift: Day 5x8-Hour (08:00 - 16:30)
Location: Castro Valley, CA
Duration: 13 weeks

**Nurse (Stroke) Navigator**

Job Accountabilities:
  • Supporting patients throughout the healthcare process, from detection to treatment and beyond.
  • Educating patients about their diagnoses, treatments, and the side effects of these.
  • Providing resources for patients seeking reliable information.
  • Helping patients to overcome barriers such as financing or transport.
  • Referring patients to Social Workers or Counselors, as needed.
Mandatory Skills
Requirements:
  • CA RN
  • BLS-Basic Life Support.
  • 5 years recent relevant acute care experience for stroke patients or as a nurse navigator assisting patients with post discharge babrriers
  • Comfortability with chart review to identify gaps in stroke care and ability to close the loop with the primary nurse in a collaborative manner.
  • Effective communicator related to stroke education to both staff and patients.
